The BNI has an interesting 'free market' system where the operating rooms run a 'first come, first served basis'. So the nurses arriving early can basically pick and choose which neurosurgeon they work with that day. Thereby the system encourages the neurosurgeons to build a strong relationship with the nurses so that they (the neurosurgeons) are picked regularly.</image:caption>

Sylvester comments on Spetzler's legendary powers of concentration. 'Concentration is the hallmark of a neurosurgeon, Spetzler's ability to entirely focus himself and his ability to suddenly then say, 'OK I've gotten to my point in this neurosurgery, I'm going to leave for 20 minutes' and he'll scrub out and his mind is entirely on something else, whether it's another neurosurgery or a hiring or the budget, he can entirely take himself away and then entirely bring himself back.</image:caption>
